Inspector’s note: Observed opening at base of front entrance outer door and at base and top left of rear exit door leading to dumpster area. Instructed to seal all outer openings to prevent pest entrance.

Inspector’s note: Observed pallets and pieces of wood used for elevation in liquor walk in cooler and boxes stored on pallets or floor on sales floor, in cashier area, in rear storage and rear liquor storage near office. Instructed to eliminate usage of pallets and pieces of wood for elevation throughout facility, instructed to provide adequate shelving units to elevate all food and nonfood items at least six inches off of floor.

Inspector’s note: Observed damaged gaskets on doors 4 and 5 of cold holding wall display unit on sales floor. Instructed to repair or replace damaged gaskets on doors 4 and 5 of cold holding wall display unit and maintain.

Inspector’s note: Observed no service sink in facility. Instructed to install a conveniently located service sink that shall have an adequate supply of hot and cold water under pressure and properly connected to the city of chicago water supply equipped with a floor drain for the cleaning of mops and for the disposal of mop water or similar liquid waste.

Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: Observed no covered receptacle in all gender employee washroom. Instructed to provide covered receptacle for all gender employee washroom for sanitary products.

Inspector’s note: Observed door of employee all gender washroom to not be self closing. Instructed to install self closing device on employee all gender washroom door so that it shall be tight fitting and self closing.

Inspector’s note: Observed opening around light switch/outlet cover in all gender employee washroom. Instructed to smoothly repair opening around light switch/outlet cover in all gender employee washroom. Observed stained ceiling tiles and stained ceiling throughout facility. Observed stained ceiling tiles on sales floor and stained ceiling in liquor storage room in office. Instructed to replace all stained ceiling tiles on sales floor and repaint stained ceiling in liquor storage room in office and maintain. Observed missing floor tiles in hallway leading to rear exit. Instructed to smoothly replace all missing floor tiles throughout facility for ease of cleanability and maintain.

Inspector’s note: Observed debris and dust buildup on floors underneath all shelving units and wall display cold holding unit on sales floor. Instructed to clean all floors underneath all shelving units and wall display cold holding unit on sales floor and maintain.

Inspector’s note: Observed clutter in rear hallway leading to exit door. Clutter observed include wood and unused equipment. Instructed the premises shall be free of all items that are necessary to the operation or maintenance of the establishment such as equipment that is nonfunctional or no longer used.
