Inspector’s note: Observed boxes stored on floor in westside office, dishwashing/storage area and drinks stored on soda crates in walk in coolers. Instructed to provide adequate shelving units to elevate all food and non food items at least six inches off of floor for ease of cleanability.

Inspector’s note: Observed no stoppers for three compartment sink. Instructed to provide proper fitting stoppers for three compartment sink.

Inspector’s note: Unable to locate backflow prevention device on water line of coffee machines. Instructed to install backflow prevention device on water line of coffee machines so that it shall be located so that it may be serviced and maintained.

Inspector’s note: Observed no grease trap installed on three compartment sink. Instructed to install grease trap on three compartment sink so that it shall be located to be easily accessible for cleaning.

Garbage wasn’t handled or stored properly.

Inspector’s note: Observed clutter in enclosed dumpster area. Observed wood, umbrella, paint can and barrel stored in enclosed dumpster area. Instructed an enclosure for refuse shall be maintained free of unnecessary items.

Inspector’s note: Observed black buildup on condensor fan covers in walk in cooler. Instructed to clean condensor fan covers and maintain. Observed debris on floor, corners and along bases of wall underneath shelving units in walk in cooler. Instructed to clean floors, corners underneath shelving units in walk in cooler and maintain. Observed buildup on floor underneath shelving units and counter behind coffee and drink machines in dishwashing/storage area. Instructed to clean and maintain. Observed cardboard, shelving units and soda crates on floor in unused section of walk in cooler. Instructed to clean unused section of walk in cooler and maintain.

Inspector’s note: Observed damaged counter underneath fountain machine, observed broken door underneath coffee machine, observed opening between wall and baseboard to the right of toilet, observed opening between wal and handwashing sink behind faucet at handwashing sink in washroom.observed damaged counter surrounding two compartment sink in cashier enclosure. Instructed to repair in said areas and maintain.
