Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: 5-501.17 observed no covered receptable inside of the toilet rooms. Instructed to provide in all toilet rooms.

Floors, walls, or ceilings were damaged or dirty.

Inspector’s note: 6-501.114 observed items stored directly on the floor in the utility closet. Instructed to remove any unnecessary clutter and elevate everything at least 6 inches from the ground.

Insects, rodents, or animals were present, or the building wasn’t sealed against them.

Inspector’s note: 6-501.111 observed evidence of pest activity on site. Observed about 50 rodent droppings scattered throughout the facility; about 10 rodent droppings in a cubbie cabinet, about 10 rodent droppings in the food prep area, about 20 rodent droppings under the hand washing sink in the toilet room and about 10 rodent droppings in the utility closets. Instructed to eliminate pest activity, clean and sanitize affected areas and recommended contacting pest control for service. Priority foundation. Citation issued 7-38-020(a).

Dishwashing equipment wasn’t set up, maintained, or used correctly.

Inspector’s note: 4-501.110 observed the 3 compartment sink hot water not maintaining 110f. Instructed to maintain 110f at all times. Priority foundation. Citation issued 7-38-025.

Inspector’s note: Observed stained surfaces inside 3-compartment sink and inadequate stoppers for 3-compartment sink. Instructed manager to repair and maintain.

Inspector’s note: Observed over 100 mice droppings scattered on floor behind reach-in freezer in kitchen, on floor and shelves inside electrical room, utility room and dry storage areas. Inspector’s note: Observed the walls in the rear storage unfinished. Instructed to finish wall, sealing all openings and gaps. Found the rubber coving missing or unattached from rear walls in storage areas. Instructed to provide and reattach rubber coving and maintain.
