Inspector’s note: 6-501.112 observed dead small insects on the floor by the rear exit door located in the food prep area. Instructed to clean and sanitize.

Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: Observed no covered receptacle in north staff washroom. Instructed manager to provide and maintain.

Inspector’s note: Observed debris on floor along walls and corners in prep, storage, utility and classroom areas. Instructed manager to clean and maintain.

Equipment and surfaces were dirty.

Inspector’s note: Found the lower surface of the cabinets under the hand washing and 3-compartment sink in the kitchen area with dirt/debris. Instructed manager to clean and maintain cabinet surfaces.

Floors, walls, or ceilings were damaged or dirty.

Inspector’s note: Found floors in the mop sink and all other closets with dirt and debris. Floors in mop sink area are wet. Instructed manager to clean and maintain dry floors.

Inspector’s note: Found the mop stored inside of the mop bucket in the closet. Instructed manager to store mop hanging up for proper drying. Maintain same.

Inspector’s note: Found the mop sink closet area and the front storage closet area not organize. Instructed manager to eliminate clutter, organize the areas and store items 6" off the floor. Maintain same.
