Inspector’s note: Observed dead bugs on floors behind refrigerators, in corners and in window areas. Instructed to clean and maintain all areas.

Inspector’s note: Observed a leak under the toilet room sink located on the first floor rear toilet room to the right. Instructed to reapir and maintain.

Inspector’s note: Observed unused equipment up stairs in rear area (ice cream freezer) instructed to remove any unused equipment as a means of preventing possible pest or insect harboring...also observed inside of all closets upstairs in need of organizing elevating items off of the floor at least six inches. Instructed to maintain areas.
