Inspector’s note: Observed leaking faucet on 3-compartment sink. Instructed manager to repair and maintain.

Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: Observed no toilets in children washrooms. Instructed manager to properly install toilet and hand washing facilities in children washrooms. Priority foundation 7-38-010

Floors, walls, or ceilings were damaged or dirty.

Inspector’s note: Observed dust buildup inside ventilation hood, ceiling and wall vents, debris on floors along walls and corners in kitchen. Instructed manager to clean and maintain.

Ventilation or lighting was inadequate.

Inspector’s note: Observed right ventilation hood not operable. Instructed manager to repair and maintain.

Insects, rodents, or animals were present, or the building wasn’t sealed against them.

Inspector’s note: Observed 1/4" gap between doors at exit #4. Instructed manager to seal all outer openings on doors to prevent pest entry.

Inspector’s note: Observed dust buildup on fan, ceiling, and wall vents in cooking area. Instructed manager to clean and maintain all fans and ceiling/wall vents.

Frozen food was thawed the wrong way.

Inspector’s note: Observed improper thawing method used for mahi mahi fish. Observed thawed fish inside refrigerator in individually wrapped reduced oxygen packages, marked "remove before thawing." instructed must follow packaging instructions and remove fish from rop before thawing. The operator voluntarily discarded denatured fish during inspection.

Dishwashing equipment wasn’t set up, maintained, or used correctly.

Inspector’s note: Observed low-temperature dish machine currently not in use with 0 ppm of required chemical sanitizer. Instructed dish machines must be repaired and maintained at all times. Priority foundation violation 7-38-025 no citation issued.

Inspector’s note: Observed water stained ceiling tile in the hallway and in the office area; instructed to replace tile where needed.

Inspector’s note: Remove clutter from corners at various classrooms and some storage closets to prevent pest harborage and allow for cleaning and pest monitoring.

Inspector’s note: Observed leak at base of faucet of 3 compartment sink in kitchen. Must repair and maintain same. Observed water stays on for 2-3 seconds once sensor is activated in women's and men's (adult) restrooms. All faucet timers must be set to minimum of 15 seconds for proper handwashing. Must maintain same.

Inspector’s note: Observed excessive clutter on floors of all teacher's closets in childrens' rooms. Must organize all clutter and store all items 6 inches or more from the floor. Must maintain same.
