Inspector’s note: Observed no metal stem thermometer on site. Instructed person in charge to provide and maintain working metal stem thermometer for taking internal food temperatures at all times. Priority foundation #7-38-005.

Insects, rodents, or animals were present, or the building wasn’t sealed against them.

Inspector’s note: Observed no pest control log book with report and valid illinois license. Instructed person in charge that licensed pest control service is required and must retain reports on site. Priority foundation #7-38-020(c).

Dishwashing equipment wasn’t set up, maintained, or used correctly.

Inspector’s note: Observed no chemical test kit on site for measuring sanitizer. Instructed person in charge to provide and maintain chemical testing kit at all times. Priority foundation #7-38-005.

Plumbing problems, or missing protection against backflow.

Inspector’s note: Observed no service sink in rear prep area. Instructed person in charge to install service sink and maintain.

Floors, walls, or ceilings were damaged or dirty.

Inspector’s note: Observed wall above three compartment sink and wall in restroom around handsink to be damaged. Instructed person in charge to repair and maintain smooth and easily cleanable.
