Inspector’s note: Observed staff toilet door in rear not self closing. Instructed person in charge to install and maintain self closing device on toilet room door.

Inspector’s note: Observed debris and clutter on floor in rear storage room where soda bibs are kept. Instructed person in charge to clean and maintain area.

A previously cited basic violation still wasn’t fixed.

Inspector’s note: Observed previous core violations from report#2619795 dated:6/27/25 not corrected; #16-interior sides of ice machine with rust deposits. Instd to remove rust and maintain all interior surfaces clean. #58-missing allergen training for city of chicago certified managers. Instd to obtain training and maintain verification on site. Instructed person in charge to come into compliance. Priority foundation #7-42-090. Citation issued.
