Inspector’s note: Front door propped open with no screen door to prevent rodent/insect from entering in the facility. Instructed to keep door closed or provide a screen door. Maintain same.

Food wasn’t protected from contamination during prep, storage, or display.

Inspector’s note: Soap/sanitizer dispensing hose on 3-compartment sink is too long, going passed the fill line on sink. Instructed to shorten hose so as to prevent any contamination or back siphonage. Maintain same.

Staff personal cleanliness was inadequate.

Inspector’s note: Food handler/employee in food prep area and not wearing a hair restraint. Instructed to obtain and maintain wearing a hair restraint while in the food prep area.
