Inspector’s note: Observed ceiling in 2nd floor mdf room with damaged and peeling paint. Instructed person in charge to repair and maintain smooth and easily cleanable.

Inspector’s note: Observed the first floor girls restroom ceiling in poor repair(cracked and a section had fell down, instructed to repair. Also in the lunchroom employees restroom peeling paint around the hand sink, instructed to remove peeling paint, and reseal. Pipes in the prep kitchen over the stove, and behind the hood dusty, instructed to remove the dust.

Inspector’s note: Must provide running hot water to the right washbowls in both the girls/boys toilet rooms by room 125. Must increase the cold water pressure at the washbowl in the toilet room in the prep area.

No evidence of rodent or insect outer openings protected/rodent proofed, a written log shall be maintained available to the inspectors

Inspector’s note: Mice droppings noted (over 30) scattered in storage cabinets under sink and computers, along wallbase and in corners in rm 120. Clutter noted in same room. Instd to remove droppings, clean and sanitize all affected areas and organize area. Citation issued 7-38-020 serious.
