Inspector’s note: Observed approximately 20 mice droppings on site. Mice droppings were observed in the following areas/surfaces in the food prep area: on the floor near the garbage, under the food prep table, and under the post elevating the microwave. Also, in the classrooms: in the lower part of a cubby and in the lower storage compartments labeled construction paper and blankets. Instructed to remove mice droppings, clean, and sanitize all affected surfaces. Priority foundation violation 7-38-020(a), citation issued.

Inspector’s note: Observed no trash receptacles provided. Instructed to provide lids on the trash can in the restrooms used by employees.

Instructed to eliminate mouse activity and clean affected areas. Recommended pest control service facility on or after 10/16/2023. Priority foundation violation 7-38-020(a) citation issued.

Inspector’s note: Observed nonfood items stored on floor in classroom. Instructed to provide adequate shelving to elevate all nonfood items at least six inches off of floor for ease of cleanability and to prevent areas for pest harborage.

Inspector’s note: Observed dust in lower cubbies in classrooms throughout facility. Instructed to detail clean and maintain. Observed dust behind all bookshelves and cubbies along bases of wall throughout facility. Instructed to clean behind all bookshelves and cubbies and maintain.

Inspector’s note: Observed missing cover next to handwashing sink in women's washroom on heater along wall. Instructed to replace missing cover on heater and maintain.
