Inspector’s note: Observed cracked and broken ceiling tiles on second floor near restrooms. Instructed person in charge to replace and maintain.

Inspector’s note: Observed 1/4" gap on bottom right corner of overhead door in storage area, 1/4" gap at bottom of north exit doors near kitchen. Instructed manager to seal all outer openings on doors to prevent pest entry.

Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: Previous core violation not corrected from report #2546810 on 1-24-22: west double entrance doors and south double entrance doors not rodent proof, approx 1/4" gap in between doors. Instd to repair so as to be tight fitting. Priority foundation 7-42-090 citation issued

Premises maintained free of litter, unnecessary articles, cleaning equipment properly stored

Inspector’s note: Any unnecessary clutter in storage areas, janitor closets...etc in need of organizing and or removing of clutter. Must correct and maintian at all times.
