Insects, rodents, or animals were present, or the building wasn’t sealed against them.

Inspector’s note: Observed openings at base of both outer front entrance doors on westside of facility and observed openings in middle at base of double dock doors in dumpster area. Instructed to seal all outer openings to prevent pest entrance.

Clean utensils, equipment, or linens weren’t stored and handled properly.

Inspector’s note: Observed beverage display containers, bike, ladder, soda crates, heater and fan stored in washroom. Instructed equipment may not be stored in toilet rooms. Instructed to remove items from toilet room to prevent contamination.

Equipment or surfaces were in poor repair or made of material that can’t be cleaned.

Inspector’s note: Observed chemicals stored on floor underneath and on sides of three compartment sink, boxes stored on floor in rear storage area, and containers stored on floor on sales floor. Instructed to provide adequate shelving units to elevate all food and non food items at least six inches off of floor for ease of cleanability.

Floors, walls, or ceilings were damaged or dirty.

Inspector’s note: Observed spills and food debris on interior of lower cabinets underneath coffee cabinets and soda stations. Instructed to clean interior of lower cabinets underneath coffee cabinets and soda stations and maintain.
