Inspector’s note: Observed heavy equipment on the 1st floor serving area in need of cleaning. Instructed to remove food debris on steam/prep table shelving. Must be maintained while not in use.

Inspector’s note: OBSERVED EVIDENCE OF PEST ACTIVITY ON THE PREMISE DURING THE TIME OF INSPECTION. NOTED APPROXIMATELY 4 LIVE COCKROACHES IN PREP AREA ON FLOOR NEAR HOT WATER TANK.RECOMMENDED TO CONTACT PEST CONTROL FOR SERVICE. MUST ELIMINATE PEST ACTIVITY. PRIORITY FOUNDATION VIOLATION. #7-38-020 (a). CITATION ISSUED.

Inspector’s note: Observed build up of grime on bottoms of wall as well as floors behind cooking equipment. Instructed person in charge to detail clean and maintain.

Plumbing problems, or missing protection against backflow.

Inspector’s note: Observed a leak at the faucet base of the three compartment sink located in the rear of the food prep area. Observed the faucet aerator in the rear prep at the hand washing sink spraying water (no steady flow) repair as needed. Observed the faucet heads in the boys toilet rooms loose (check all and tighten or repair as needed) observed the stall at the entrance of the boys toilet room on the first floor coming loose from wall. Instructed to repair all areas.

Inspector’s note: Observed the walls and floors under cooking and non cooking equipment with a build up of dust, dirt and small debris. Instructed to clean and maintain all areas.

Inspector’s note: Observed multiple items not used in kitchen area tagged does not work, do not use and a dish washing unit not being used. Instructed they must!!! Remove any and all unused equipment as a means of preventing possible pest and or insect harboring.
