Inspector’s note: 6-201.13 observed wall base missing in prep area to the left of refrigeration unit. Instructed to replace and maintain.

Thermometers were missing or inaccurate.

Inspector’s note: Observed no metal stem probe thermometer on site during time of inspection for testing internal temperature of tcs foods (milk, yogurt, deli meat). Instructed to provide and maintain on site at all times. Priority foundation violation. #7-38-005. Citation issued.

Inspector’s note: Observed evidence of pest activity on premise during time of inspection. Noted approximately 15 small black live flies in prep area on towels, walls, sink basin, countertops and cooling unit surfaces, one live adult cockroach behind entry door of classroom before reaching rear exit door of facility, and approximately 35 small live ants in baby room underneath lined baby cribs recommended to contact pest control service for service. Must eliminate pest activity on premise and maintain. Priority foundation violation. #7-38-020 (a). Citation issued.

Inspector’s note: Observed outside garbage receptacle lid not tight fitting per city code. Instructed to repair or replace dumpster lid, to prevent rodent harborage. Instructed to maintain. Priority foundation 7-38-020(b) citation issued.

Inspector’s note: Observed food residuesa and build up on the oustide of refrigerator and the kitchen floor. Instructed to detail clean and maintain all cooking equipment, racks and shelving and floor throughout. Recommended to increase cleaning frequency and maintain at all times.

Inspector’s note: Observed pest activity with approximately 30 large flies noted during inspection. Large fly activity was noted near the rear exit door and throughout the facility and in classrooms. Advised to consult pest control as additional services are needed to minimize or eliminate activity prior to re-inspection. Priority foundation violation #7-38-020(a). Citation issued.
