Inspector’s note: Observed evidence of pest activity on premise during time of inspection. Noted approximaltely 38 small rodent droppins in the following places in prep area under 3 compartment sink in corner, following along the wall underneath grey piping, alongside utility sink in rear toxic storage are, scattered on floor around washing machine, under table storing microwaves, along the west side window on floor, scattered on floor inside all corners of the "clubhouse house". Reommended to contact pest control for additional service. Must eliminate pest activity. Priority foundation violation. #7-38-020 (a). No citation issued.

Floors, walls, or ceilings were damaged or dirty.

Inspector’s note: Observed chipped paint on the ceiling above prep area in the kitchen and from the metal duct pipe above 2 compartment sink. Instructed to resurface and repaint chipped areas to ensure ceiling and metal duct pipe are smooth and easily cleanable.

Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: Observed no covered waste receptacles for sanitary products in restrooms for female use. Instructed person in charge to provide covered waste receptacles in all restrooms for female use.

Garbage wasn’t handled or stored properly.

Inspector’s note: Observed outside garbage dumpster in poor repair.observed lid broken and another one pushed down inside of dumpster leaving it not tight fitting.must repair or replace to be tight fitting.(priority foundation 7-38-020(b)

Equipment or surfaces were in poor repair or made of material that can’t be cleaned.

Inspector’s note: Noted torn rubber gasket inside the 2-door cooler 2nd from serving line. Must reseal/replace.

Inspector’s note: Observed water leaking from several areas of utility sink faucet in rear closet behind lunch room. Observed several towels piled on top of faucet to subdue leaking. Must repair all leaks at faucet of utility sink. Must maintain same. -

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. REPAIR TOLIET /LEAKY IN POOR REPAIR IN GIRLS WASHROOM.
