Inspector’s note: 6-202.15 observed 2nd floor exit door outer opening not protected. Instructed to repair door sweep in order to prevent pest entry.

Inspector’s note: 4-601.11(c) out c 10/18/2025 observed equipment in not in use bar in rear as well as 2nd floor in need of cleaning. Must remove accumulated food debris from interior and exterior cooling equipment, bar top liquor wells and sink basins in order to prevent pest harborage in said spaces.

Inspector’s note: 5-205.15 observed not in use bar 2nd floor sink not draining during time of inspection. Instructed to reapir and maintain plumbing in good condition.

Instructed person in charge that a licensed pest control company and operator must provide at least 2 pest inspections a year and the reports must be maintained in the log book. Inspector’s note: Observed debris on floors behind both bar areas and on stairs and floors leading to upstairs second floor bar. Instructed person in charge to clean and maintain.

Inspector’s note: No pest control log book. Found no pest control log book from a licensed company with the necessary items inside of book; must provide. Instructed to obtain and have on premises at all times. Priority foundation violation 7-38-020(c).

Inspector’s note: Found several outer doors not completely rodent proofed as required on the first and second floors; must completely rodent proof all doors to prevent possible rodent entry.

Inspector’s note: No chemical test strips on premises as required; must provide. Found no chemical test strips for testing the sanitizer when wash rinse and sanitizing glasses; must provide the appropriate test strips for the sanitizer used. Priority foundation violation 7-38-005.

Inspector’s note: No utility sink on premises for disposing of waste water; must provide. Instructed to install a utility sink on premises for disposal of waste water with hot and cold running water and a backflow device attached to the water line.

Inspector’s note: Found no backflow prevention device on the large ice machine by the front bar; must provide. Must install a backflow prevention device to be serviced and maintained.

Inspector’s note: Observed an opening in the floor behind the bar on the second floor surrounding some pipes; insructed to seal opening in the floor.
