Inspector’s note: Observed leak underneath 3 compartment sink and the pressure at hot water faucet in green room is low. Instructed manager to repair and maintain.

Toilet rooms weren’t built, stocked, or cleaned properly.

Inspector’s note: Observed missing cover on waste receptacle in staff toilet room. Instructed manager to provide and maintain.

Inspector’s note: Observed unused equipment and unnecessary articles in basement storage area. Instructed manager to remove all unused, unnecssary equipment and articles to prevent pest harborage.

Inspector’s note: Found the first and last washbowl (4 total) in the children's toilet room in poor repair. The first one has no cold water and very low hot water pressure. The last one has no running water at all. Instructed to fix and maintain plumbing. -found water squirting up from the base of the faucet when water is turned on at the 3-compartment sink. Instructed to fix and maintain.

Inspector’s note: Found the trash receptacle in the staff's toilet room without a lid/cover. Intructed to provide a trash receptacle with a lid/cover and maintain.

Inspector’s note: Found the wall around the mopsink in the utility room missing tiles. Instructed to provide and maintain. -the floor tile at the threshold of the entrance to the rear hallway is broken/damaged. Instructed to fix or replace floor tile. Maintain same.

Inspector’s note: Observed the floor in the basement staff room not clean, instructed to clean.also replace missing wall coving under vent in same above room. Observed a black substance on the ceiling and wall in the basement sewer room, instructed to clean, and sanitize/ or replace wall.
