Inspector’s note: Observed cracked ceiling tile(s) and gaps in the ceiling tile(s) in the infant classroom. Instructed to replace/repair ceiling tile(s), clean and maintain.

Observed missing lightbulbs or lightbulbs out in the infant classroom. Instructed to replace with working lightbulbs. Observed in the explorers classroom three recessed light fixtures in need of replacement/repair and one fixture missing a lightbulb cover/shield. Instructed to provide/replace replace.

Inspector’s note: Observed vents throughout the facility to be dirty with debris/dirty lent specifically the vents in the staff washroom, infant classroom, sunshine room, explorers room shooting stars room. Instructed to clean and maintain.

Inspector’s note: Observed the pest control log book not maintained. Only one pest inspection was performed in the past year. Management instructed that to have at least 2 pest inspections per year from a licensed pest control company. Priority foundation violation 7-38-020(c). Citation issued.
